Forms for FBC
* For use of building space, please submit a use form at least 1 week in advance of your event.
* A Bldg/Equipment use form must be returned to the Church Office and approved by Office Staff before use of space or equipment can occur.
* If you call the Church Office to reserve a date for space/equipment use, a tentative “hold” will be put on the items you request.
* If a use form is not submitted within 48 hours of your request, the “hold” will be removed. (Completed forms may be submitted in person, by fax or by e-mail. Office hours are Mon.-Fri., 8:30 a.m. - 4:00 p.m., Monday – Friday.)
[Please Note: Church property may not be removed from the building without an approved use form.]
